A new program called LifeCourse has been created by Allina Health to fill the gaps across the health care spectrum. LifeCourse will serve as a care guide to provide patients with serious illnesses in the late stages of life. Care guides allow people to manage their care and be more vocal about what they want out of their care.
LifeCourse has only been around for a short period of time, however the project has discovered that a person’s quality of life can be maintained or improved when working with a care guide. The positive results of LifeCourse has expanded its reach in 2016 to primary care settings, specialty clinics and neighborhood services. For the full article, click here.
