In a letter to Seema Verma, the Administrator of the Centers for Medicare and Medicaid Services (CMS), the authors of the Patient Choice and Quality Care Act ask that CMS test some of the new models of care for patients with advanced illnesses that are outlined in their legislation.
The Patient Choice and Quality Care Act – enables older adults with advanced illnesses to have access to the full range of high-quality medical care and treatment, including curative care, palliative care, and hospice care. Legislation sponsors Rep. Earl Blumenauer (D-OR), Rep. Phil Roe (R-TN), Sen. Mark Warner (D-VA), and Sen. Johnny Isakson (R-GA) believe the CMS has the statutory authority to begin testing some of these new models under section 1115A of the Social Security Act.
The legislation calls for the development of an Advanced Illness Coordination Services demonstration that would allow an interdisciplinary team to provide early palliative care and wrap-around, home-base services to individuals with multiple and complex chronic conditions. The demonstration would also require at least 1 face-to-face encounter with a patient, family caregiver, or legal guardian, to discuss the patient’s typical illness trajectory and to help define goals of care, care preferences and treatment options.
By providing these palliative care services, legislation authors believe the demonstration will:
- Reduce unnecessary emergency room visits and hospitalizations
- Ease family and caregiver burden
- Improve quality of life for individuals with serious and advanced illness
