New Requirements for Discharge Planning Proposed by CMS

CMS issued a proposed rule to revise the discharge planning requirements that ACH, LTCH, IRF, CAH and HHAs, must meet in order to participate in Medicare and Medicaid programs. The proposed rule was mainly issued to implement the discharge planning requirements in the IMPACT Act. The Act requires CMS to modify the CoPs and subsequent interpretive guidance to require providers to take into account quality, resource use, and other measures in the discharge planning process. The discharge planning regulations must address the settings in which a patient may be discharged and stresses the importance of taking the patient’s goals and preferences into consideration as part of the discharge planning process.
