The Future of Home Health project, initiated by the Alliance for Home Health Quality and Innovation, demonstrates the changes that the home-based care must develop.

The data from this report was summarized by an independent research firm called Avalere Health, which included interviews with key stakeholders, patients and caregivers, policy-makers and payers to address the future for home health care.

September 30-October 1, 2014, “The Future of Home Health Care” was held to identify the key issues for future focus with the creation of this report.

Home health care is positioned as the “triple aim” for the following Affordable Care Act reforms goals:
• Improving patients’ care experience
• Improving the health of populations
• Reducing costs

These are some of the characteristics of a next-generation home health agency, according to Alliance board vice chairman and VNA Health Group President and CEO Steve Landers, M.D.:
• Patient-centered
• Connected
• High quality
• Tech-enabled

In the future, home health agencies will serve critical roles in the following areas:
• Post-Acute Care and Acute Care Support
• Primary Care Partners
• Home-Based Long-Term Care Partners

At “The Future of Home-Based Care,” on June 21, 2016 in Cincinnati, Ohio, the authors acknowledged the support from Corridor and several other organizations that provided funding for the Future of Home Health project.

Leadership from multiple stakeholders, including patients, caregivers, policy-makers, payers, and providers will be the fundamental element to transform the home health and home-based care.
