Hospice Quality Reporting is designed to assist in the promotion of patient-centered, high quality, safe care delivered by hospices.
CMS has set forth a set of measures that have been vetted and recommend by multi-stake holder organizations across the country and finalized with the assistance of many providers, payors and other stakeholders.
Today, there are two components to Hospice Quality Reporting and all Medicare Certified hospice organizations must comply with the two reporting components:
1. HIS or Hospice Item Set – which includes HIS data gathered at admission on all patients regardless of payor source, age or location of services. Data is collected at discharge as well. Individual scores on each of the seven (7) data elements publicly reported.
2. HCAHPS or Hospice Consumer Assessment of Health Care Providers and Systems – consumer data gathered by an approved CAHPS vendor and provided to the organization which measures experiences the patients and their caregivers have with hospice.
To be compliant with this requirement, hospices must submit the required data. For those hospices that fail to submit the required quality reporting data, their market basket update be reduced by 2% in FY2017. The submission threshold currently for FY2018 is 70%, increasing to 80% in FY2019 and 90% in 2020 and beyond.
The gathering and submission of the above described data is in response to the Affordable Care Act requirements to report quality measures of care provided to hospice patients and make the information publicly available on the CMS website. This section within the CMS website will be identified as Hospice Compare. The design and development of this site is currently underway with plans to release access mid-summer of 2017. Plans for the site are to allow consumers and providers to view data on all Medicare Certified hospice providers serving their region, city and/or zip code. The HIS quality data and HCAHPS data results will be readily available for consumer viewing and comparison. In time this reporting site will also feature a quality rating system very similar to the home health 5 Star Rating.
CMS offers Hospice Quality Reporting training to ensure each hospice understands the requirements for data collection and submission, how to submit the data, and preview available reports.
