CMS Proposes Clinical Template For F2F Encounter

CMS has issued a notice for the home health face-to-face (F2F) electronic and paper clinical template. The template is a progress note that was developed to help physicians document the home health F2F encounter required for Medicare certification of home health patients. Use of the template is voluntary. The template contains a combination of checkboxes and blanks for narrative documentation. The checkboxes relate primarily to the physician’s physical assessment of the patient. Documentation related to the reasons for skilled services and homebound requires a narrative explanation that is similar to the old F2F encounter document that agencies used when a narrative was required for F2F. NAHC has concerns that the template contains language that is misleading and that it will not be received well by physicians due to the template having a format that essentially adds the F2F narrative requirement back into the process for physicians certifying home health patients.
