This webinar, in collaboration with Administration for Community Living, will describe practical person-centered tools and approaches organizations and professionals can adapt to better support aging in place. Person-centered care is consistent with the values, needs, and desires of individuals and is achieved when providers and health plans involve individuals in healthcare discussions and decisions. Speakers will share strategies for integrating person-centered approaches into clinical and community-based settings and will discuss how approaches differ based on the individual goals of the client.

Training Opportunity: Identifying and Meeting the Language Preferences of Health Plan Members
This webinar will provide an overview of strategies health plans can use to assess and meet diverse language preferences. Health plans serving dually eligible beneficiaries can play a key role in identifying and communicating language preferences to front-line staff and providers in their network to help remove language barriers to care. Speakers will share strategies for collecting language preference data and examples of how plans use this data to inform health plan operations, such as providing translation and interpretation services. Speakers will also share strategies for hiring and training health plan staff to provide linguistically and culturally competent services.
