Interdisciplinary Teams Provide Support in Home Health Care

At times, providing care for a home health patient can be difficult. Once patients move from an acute setting back to their homes, it is important that professionals communicate throughout the entire process. A session was held to prepare patients receiving in-home care titled, “Interdisciplinary Approaches: What Home Health Providers Can Learn from Hospice IDTs about Patient Care and Cross Continuum Transitions.” The session stressed the importance of setting goals and outlined care models in different settings. In order for the patient to receive care properly, an agency and its entire staff need to communicate with monthly meetings to ensure the patients are comfortable going home.
