The Office of Management and Budget (OMB) has approved the renewal of the Home Health Change of Care (HHCCN).
Beginning July 1, 2019, all Home Health Agencies will be required to use the new form which has an expiration date of April 30, 2022. HHAs can continue to use the old for until July 1, 2019, but they encourage agencies to start transitioning to the new form as soon as possible. The new form is identical to the old one, minus the new expiration date.
